MAT Community Outreach (Rural Areas)

We are expanding access to safe, structured outpatient care through Medication‑Assisted Treatment (MAT) for individuals living in the rural communities surrounding Madison. Our goal is to bring support closer to home by offering monthly in‑person outreach days in Switzerland County, paired with ongoing virtual follow‑up care.

What We Do

Expanding Access to Safe, Structured MAT Care

Access to opioid treatment shouldn’t depend on transportation, distance, or the size of your town. Our MAT Community Outreach program brings safe, evidence‑based care closer to individuals living in rural Indiana counties.

Whenever possible, I prefer to see patients in person.  We are establishing monthly in‑person outreach days in Patriot, Indiana at this time.

Our outreach model is built around:

  • Structured, medically supervised treatment

  • One‑on‑one counseling and support with the provider

  • Consistent follow‑up, whether virtual or in person

  • Clear expectations and accountability

  • Community support resources provided to every patient

This approach allows us to deliver care while reducing barriers for individuals in rural areas.

Where We Serve

Rural Southeastern Indiana 

We are expanding outreach throughout Southeastern Indiana, with a focus on communities that have limited access to treatment options.

Switzerland County:

If you live in Patriot, East Enterprise, Florence, or rural Switzerland County and have difficulty accessing care, please contact our office. We plan to be in-person minimum of once per month in PatriotVirtual visits are available. The goal of virtual visits will be for those visits that are between our in-person visits.

Our Community Outreach program is designed for individuals who struggle with opioid dependence, and:

  • Live in rural counties with limited treatment options

  • Face transportation barriers

  • Need consistent, structured MAT care

  • Prefer a small, private, non‑corporate setting

  • Want one‑on‑one support directly from their provider

How to Get Started:

If you are interested in MAT services through our Community Outreach program, call our office 812-265-2500. We will gather your information and determine whether our setting is the right fit. If we are not able to meet your needs, we will help direct you to other local resources.
